Reporting

In order to receive continued funding, grantees must complete progress reports which are housed in your online account and described below.

One year after receipt of a grant, please complete the appropriate progress report. Your account will indicate the correct report, either an operating or Capital Grant report. These progress reports will serve as annual and final reports for multi-year grants, and the final report for single-year grants.

Please note the due date to be sure you are completing the correct report. Your online account may be accessed by clicking on the link below. Once you enter the user name and password associated with your account, click on the tab labeled “Requirements” and you will see the appropriate form.

Grant Budget Form

To be used for (1) a multi-year program grant, or (2) a one-year grant with multiple components.

Download the form and upload as an attachment to your corresponding online grant application. Click here to log into your account and upload.

Quarterly Fundraising Report

For updating the Foundation on the status of capital or project campaigns, if requested by the Foundation.

Download the report and email  the completed report as a PDF to Emily Dukes, Program Associate, copying Courtney Latta Knoblock, Program Director.

Annual Scholarship Report

To be used by universities with Anne and Henry Zarrow Scholarship Programs for their annual reporting requirement.

Download and complete a separate form for each Scholarship Fund or Endowment. E-mail the form(s) to: Laura Blais, Grants Administrator. Contact Laura with any questions by e-mail or at 918.295.8008.
